NRC INSPECTION REPORT 50-315/99011(DRS); 50-316/99011(DRS)

Dear Mr. Powers:

On April30, 1999,.the NRC completed a special inspection at your D. C. Cook facility. The

enclosed report presents the results of that inspection.

The purpose of the inspection was to review the circumstances surrounding two electrical faults

associated with motor control centers 2-AM-8 and 12-TSC-S which occurred on April 19, and

April 24, 1999, respectively.

To evaluate the engineering implications associated

with these

events, the inspector focused on the licensee's root cause investigation of the failures, the

operability of the electrical distribution systems required to support the current cold shutdown

plant condition, and the licensee's engineering-related

corrective actions.

Following a review of these events, the NRC determined that the root cause investigations for

the electrical faults were promptly initiated. The findings indicated that a contributing cause for

the electrical fault in motor control center 2-AM-Bwas excessive dirt and dust in the breaker

cubicle due to inadequate cleaning; and that the electrical fault in motor control center 12-TSC-S

was caused by a random age-related failure of the control power transformer.

In addition, the NRC concluded that planned long-term corrective actions to address the motor

control center failures, which included extensive breaker refurbishments and motor control

center cleaning activities prior to plant restart, were thorough and comprehensive.

However, the

NRC identified a number of nonsafety-related

breakers important for the current plant condition

which had not been prioritized for refurbishment and cleaning.

Finally, since the simultaneous failure of circuit breakers due to faults in redundant divisions was

remote, and previous surveillance testing results were acceptable,

the NRC concluded that

there was reasonable

assurance

that the electrical distribution systems required for cold

shutdown plant operation were capable of performing their function.
