Indiana Michigan Power Company Docket No. 50-315 As a result of the inspection conducted on January 19-21, and 27-28,
- 1988, and in accordance with the "General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:

Cook Operations equality Assurance
- Program, requires that activities affecting quality be prescribed by and performed in accordance with documented procedures.
Contrary to the above,'n May 19, l985, the maintenance department replaced a
Unit 1 containment isolation butterfly valve, number CCM-451, manufactured by Centerline, with a similar valve manufactured by Pratt, without first
. processing a Request for Change required by Section 5.2 of PMI 5040.
This is a Severity Level V violation (Supplement I).
The inspection showed that action had been taken to correct the identified violation and to prevent recurrence.
Consequently, no reply to the violation is required and we have no further questions regarding this matter.
