Dear Dr. Murley:
Enclosed are 21 copies of Revision 5 to the Steam Generator Repair Report for the Donald C.'ook Nuclear Plant, Unit No. 2.
Revision 5
is made up entirely of Supplement 4 which contains our response to an NRC request for additional information.
The request for additional information was made by the NRC staff during a September 15,
- 1987, meeting at the NRC offices in Bethesda, Maryland.
Attachment 1 to this letter is Revision 5 to the Steam Generator Repair Report with insertion instructions.
Attachment 2 is a Document Control Acknowledgement Letter.

In addition, as discussed in Section 6.2 and Supplement 1 of the Steam Generator Repair Report, the Indiana Michigan Power Company has decided to procure a second 150 ton capacity single-failure-proof crane to facilitate the handling of heavy loads in the auxiliary building.
A final seismic analysis of the new crane has been completed.
This final seismic analysis, which confirms the conclusions of the preliminary seismic analysis

T. E. Murley AEP NRC:0980G provided in Supplement 1 of the Steam Generator Repair Report, demonstrates that the allowable stresses of the principal structural components of the new crane are not exceeded during a seismic event with a 60 ton load on the main hook.
In compliance with the requirements of 10CFR50.91(b)(1),
copies of this letter and its enclosures have been transmitted to Mr. R.
C.
Callen of the Michigan Public Service Commission and Mr. G.
Bruchmann of the Michigan Department of Public Health.
