SUMMARY
MEETING HELD ON NOVEMBER 14 AND NOVEMBER 15, 1985 WITH IMEC TO DISCUSS WRONG UNIT/WRONG TRAIN EVENTS A meeting was held at the Donald C.
Cook Nuclear Plant between members of the NRC and staff from the plant and corporate offices to discuss wrong train/wrong unit events.
These are events when an operator performed some activity incorrectly by taking the action on the wrong train of a system or on the wrong unit.
The meeting was requested by the NRC to obtain information for use in developing guidance for the industry in preventing such events.
The meeting was attended by D. Wigginton, D. Persinko, Ann Ramey-Smith, and Jim Heller.
The licensee was prepared with a detailed presentation including samples from the tagging and labeling program underway and photographs of the areas involved in the wrong train/wrong unit events.
In addition to the labeling problems as a source of incorrect action, the licensee has also undertaken efforts to correct,
- separate, and update procedures to prevent reoccurrence of the events.
Discussions were also held with the auxiliary operators and technicians that performed the events in order to determine the root cause of the errors.
The staff was very satisfied by the information presented and the individual discussions with the operators.
