A~nndix A NOTICE OF VIOIATION American Electric Power Service Corporation Indiana and Michigan Power Company Docket No. 50"315 As a result of the inspection conducted on July 16-18, and 20,
- 1981, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix J, Section III.A.lstates in part that "If during a
Type A test....potentially excessive leakage paths are identified which will interfere with satisfactory completion of the test...the Type A test shall be terminated and the leakage through such paths shall be measured using local leakage tes'ting methods."
l Contrary to the above, three potentially excessive leakage paths, one at a flange connection on valve ECR-33, one involving stem packing on valve GCR-314, and one in isolation check valve CA-181N, were identified and repaired without performing the required measurements.
This is a Severity Ievel V violation (Supplement I).
Pursuant to the prove.sxons of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
,(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as
- amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response, time for good cause shown.
