Dear Mr. Keppler:
This letter provides the required response to the notice of violation attached as Appendix A to Mr. R. F. Heishman's letter dated July 10, 1980 and received on July 15, 1980.
The corrective actions taken in response to the item in the notice, classified as an infraction, follow:
CORRECTIVE ACTIONS The review documents are issued as a single document with only one cover sheet.
All required individuals must sign to indicate review before the document is sent back to the Training Department.
These individuals may not all be available at the time of issue due to vacation, sickness, off-site simulator training, or other special assignments.
In this case, the document is allowed to remain as an open item.
The Shift Operating Engineer ultimately follows the matter up and insures that review is docu-mented.
The importance of such followup has been stressed to the Shift IV SOE.
In the present
- case, required reviews will be brought up to date by August 15, 1980.
