NOTICE OF VIOLATION erican Electric Power
'. Service Corporation indiana and Michigan Power Company Docket No. 50-315; PO-316 Based on the inspection conducted from October 6 -November 30, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 and 2 are infractions and apply to Unit 2 only.
1.
Technical Specification 6.8.1 requires implementation of procedures.
PMP 6010 RAD.002 states in part:
"Each 'High Radiation Area'oor shall be secured by lock and key...".
Contrary to the above, door ij370 (To Unit 2 containment vestibule area) was found propped open by an Argon purge hose bring used for welding in the vestibule area.
The door was unguarded and uncon-trolled.
2.
Technical Specification 6.8.1 requires implementation of procedures.
PMI-2275 states in part:
"The 'Welding, Burning and Grinding Permit'hall be completed in duplicate.
After concurrence by the SOE...
one copy shall be posted in the work area."
Contrary to the above, the welding job referenced in //1 above had no evidence of an active welding permit at or near the work area.
