NOTICE OF VIOLATION American Electric Power Service Corporation Docket No. 50-315 Docket No. 50-316 Based on the results of the NRC inspections conducted on May 30-June 8,

June 12-15, June 25-July 2, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as noted below.

This item is an infraction.

Technical Specification 6.8.1 states in part, that, "Written procedures shall be established, implemented Indiana and Michigan Power Company Procedure No. MHP-5050-SPC-001, Revision 3, Section 3.6.4, states in part, that, "All unused rod shall be returned to the crib at the end of shift Section 3.11 states in part, "return all unused rod to crib attendent."

Indiana and Michigan Power Company Procedure No.

12MHP-SP001, Revision 3, Section 4.'7 states,.

"Extreme care must be taken to prevent the entry of foreign material into the steam generator of the feedwater system."

Contrary to the above, the inspector observed on June 7, 1979, a

container of 3/32" 7018 electrodes in steam generator 2-4 work area.

These electrodes were not in a rod heater nor could they be accounted for.

On June 13, 1979, this same container was observed lying on the grating in front of feedwater pump No. 3.

Also on June 13, 1979, portable heater No. 703-5 containing 3/32" 7018 electrodes was observed unplugged and cold in steam generator No. 1-2 nozzle repair work area.

On June 28, 1979, portable heater No. 703-16 containing 7018 electrodes was observed unplugged and cold at steam generator No. 1-3 nozzle repair work area.

In addition, the inspector observed, that control of welding materials in several other areas of the plant was not in compliance with procedure requirements.

Contrary to the above, on June 7, 1979, the inspector observed metal

chips, and grinding particles in all steam generators.

No attempt had been made to plug the generators.

The licensee was informed of this condition and took immediate corrective action by installing air bags in each nozzle.

These plugs were properly maintained throughout the remainder of the repair activities.

No reply to this item is required as the inspector has no further questions regarding this matter.
