6574 for the Model No.
HN-200 shipping cask expires on September 30, 1979.
In order that the Indiana 8 Michigan Power Company may continue to use this cask for the shipment of radioactive material, continuation as a user of this certificate in accordance with the provisions of paragraph 71.12(b) of 10 CFR Part 71 is hereby requested.
Kindly note that Mr. John Tillinghas i is no longer with American Electric Power or any of its subsidiaries.
