A endix A NOTICE OF VIOLATION American Electric Power Service Corporation Indiana and Michigan Power Company Docket No. 50-316 Based on the inspection conducted on February 5-9, 12-16, 20-23, 26-28 and March 1 and 2, 1979, certain of your activities were in noncompli-ance with NRC requirements, as noted below.
This item is an infraction.
Unit 2 Technical Specification 6.12.2 requires doors to high radiation areas in excess of 1000 mrem/hr be locked to prevent unauthorized entry.
Contrary to the above, on February 9,
1979 it was observed that the Unit 2 containment lower volume access door was unlocked.
The lower volume of the containment had areas at this time where radiation levels were greater than 1000 mrem/hr.
