Dear Mr. Keppler:
This letter is to report a loss of documentation which occurred at the Cook Plant between February 1978 and the present.
The original documentation package from the Unit 2 Ice Basket Weighing conducted on February 16, 1978 was. misplaced during the review process.
This package, included.all raw data taken during the weighing, daily scale calibrations; and the signed off procedure, 12IHP 4030.STP 211:
All final weights,.a data analysis completed-shortly after the weighing, and traceability to NBS of the weight-st'andards used are available-.
These document~ indicate that the ice inventory at the time of that.weighing:was 2.78 x 10
. lbs.
(95K confidence).
This is well above the minimum weight of 2.37 x 106 lbs-. required by the Technical Specifications.
l In addition to the above data, all participants in the February 1978 Weighing Program have signed a statement ver'ifying that the weighing was performed and all data collected in accordance with an approved surveillance procedure, "Ice Condenser Basket Weighing".
Although the original data package was misplaced, it is felt that with the data and statements available, all requirements of the D. C.
Cook Unit 2 Technical Specifications have been satisfied.

This incident occurred at a period in time when the plant technical staff was heavily overloaded with many Unit 2 preoperational test documentation packages.and, Unit 1 refueling outage surveillance. test documentation packages for review simultaneously.
With this extraordinary work load, the possibility of misplacing a data package'as significantly increased.
As this period of unusually heavy work load has ended, 'the probability of recurrence is considered to be quite small based on the fact that, under normal circumstances, the review of literally hundreds of routine surveillance tests has been and is accomplished through the normal document handling methods without problem.

The average ice weight was verified to be 2.69 x 10 lbs.

well above the 2.37.x 10 lbs. required by Technical Specifications, and all row group average weights were confirmed to be above the 1220 lb.
