SUBJECT: NRC INSPECTION REPORT 50-397/98-22 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
From October 11 through November 21, 1998, an inspection was conducted at your
Washington Nuclear Project-2 reactor facility. The enclosed report presents the scope and
results of that inspection.
During this period, three violations of NRC requirements were identified. Two of the violations
w'ere for the failure to followprocedures and one violation for inadequate procedure.
The first
violation resulted from a failure by control room operators to log the specific out of service times
for both trains of standby gas treatment system during testing, as required by plant procedures.
The second violation had two examples of a failure of maintenance personnel to perform testing
of standby gas treatment charcoal filters as required by procedures.
The third violation resulted
because your test procedure failed to ensure that an effective functional test of the charcoal
was conducted to demonstrate the system performed as designed.
The NRC has concluded that information regarding the reason for the violations and the
corrective actions taken and planned to correct the violations and prevent recurrence is already
adequately addressed
on the docket in the enclosed inspection report. Therefore, you are not
required to respond to this letter unless the description therein does not accurately reflect your
corrective actions or your position.
In that case, or if you choose to provide additional
information, you should followthe instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its
enclosures,
and your response willbe placed in the NRC Public Document Room (PDR).
