SUBJECT: NRC INSPECTION REPORT 50-397/96-24
Dear Mr. Parrish:
An NRC inspection was conducted
October 27 through December 7, 1996, at your
Washington Nuclear Project-2 reactor facility. The enclosed report presents the scope and
results of that inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
the Washington Nuclear Project-2 facility was generally characterized
by professional,
safety-conscious
operations.
In particular, we noted that your technicians demonstrated
good skills during surveillance activities.
However, we also noted that an apparent
nonconservative
decision by engineering personnel,
associated
with scheduling the repair
of the residual heat removal keepfill pump, resulted in pump failure and train inoperability.
ln addition, we noted poor communications
on the part of a licensed senior reactor
operator, combined with questionable work planning, contributed to minor injury of two
maintenance
workers.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter,
its enclosure will be placed in the NRC Public Document Room (PDR).
