REPORT 50-397/96-03

AND NOTICE OF VIOLATION

Dear Hr. Parrish:

On Harch 30,

1996,

the

NRC completed

an inspection at your Washington Nuclear

Project-2 facility.

The enclosed

report presents

the results of that

inspection.

During the 6-week period covered

by this inspection period, your conduct of

activities at the Washington Nuclear Project-2 facility was generally

characterized

by improved coordination during plant shut

down, appropriate

performance of maintenance

and surveillance activities,

some

weaknesses

in

updating the safety analysis report for long standing facility changes,

and

evidence of efforts to reduce

personnel

dose

by system flushes

and temporary

shielding.

Compared to previous reactor

shutdowns,

the conduct of the reactor

shutdown

this inspection

period

was generally better coordinated;

however,

some

weaknesses

were identified with procedural

adherence

and usage.

Additionally,

the inspectors

identified instances

during the shutdown

where operators

did

not question

the cause of alarms

and appeared

to have

an inadequate

understanding

of the alarms

and

some plant equipment

operations

We are concerned

about

two violations of NRC requirements

that were

identified.

In the first instance,

NRC inspectors

identified that operators

Failed to assure

that

a Technical Specification required chemistry

sample of

the reactor coolant

system

was taken during the shutdown process.

In the

second

instance,

NRC inspectors

identified two examples of a failure to

promptly document potentially safety-significant

problems.

These violations

are cited in the enclosed

Notice of Violation, and the circumstances

surrounding

them are described

in detail

in the enclosed

report.

Please

note

that you are required to respond to Violation A of this letter,

should follow

the instructions specified

in .the enclosed

Notice when preparing your

response,

and are not required to respond

to Violation B.

The

NRC will use

your response,

in part, to determine

whether further enforcement

action is

necessary

to ensure

compliance with regulatory requirements.
