Dear Mr. Parrish:

The U.S. Nuclear Regulatory

Commission

(NRC) performed

an integrated

assessment

team inspection of activities authorized

by NRC Operating

License

No. NPF-21 for Washington Nuclear Project

2 (WNP-2).

Special

Inspection

Branch of the Office of Nuclear Reactor Regulation

(NRR) conducted

the

inspection

from August

7 through August 17,

1995.

The team consisted of staff

from NRR and Region IV.

The team assessed

licensee

effectiveness

in identifying issues,

performing

root cause

analyses,

and implementing corrective actions.

It evaluated

the

results of the efforts to effect improvement in these

areas

over the six

months before the inspection,

with specific attention given to human

performance.

The inspection

focused

on the areas of maintenance

and

engineering.

The detailed results of the inspection

are given in the attached

aport.

Overall, the team determined that

a good self-assessment

capability had

been

developed at WNP-2.

The

WNP-2 problem identification program, utilizing

Problem Evaluation Requests,

was comprehensive;

however,

some weaknesses

in

the program were noted

and brought to your attention.

The mid-cycle self-assessment

in February

1995 had identified human

performance

problems

as

an important contributor to unexpected

events.

Such

problems

had continued to occur during the six months before the inspection.

For example,

performance-related

mistakes

caused

a feedwater transient,

a

turbine-generator trip, and another feedwater transient that

came close to

becoming

a reactor trip.

In addition, twice in thzee

months the

same high-

high area radiation door

was left unlocked

and unattended.

The team recognized that considerable

resources

had

been invested in such

initiatives as the business

plan,

Human Factors Coordinator,

Peer

Review Team,

and the Corrective Action Review Board to improve

human performance.

However,

'veness

was not yet evident

based

on their recent initiation.
