INSPECTION REPORT 50-397/95-03

AND NOTICE OF VIOLATION

This refers to the inspection

conducted

by Mr. C. J.

Paulk,

and those listed

in the enclosed

report. of this office on February

13 through March 14,

1995.

The inspection included

a review of activities authorized for your Washington

Nuclear Project-2 facility.

At the conclusion of the inspection,

the findings

were discussed

with you and those

members of your staft identified in the

enclosed

report.

A subsequent

exit meeting

was conducted

by

Messrs.

E.

E. Collins,

C. J.

Paulk,

and myself on June 7.

1995.

by telephone

with you and members of your staff to discuss

the results of our evaluation of

information provided by you at the March 14.

1995. exit meeting.

Areas examined during the inspection are identified in the report.

Within

these

areas,

the inspection consisted of selective examinations of procedures

and representative

records,

interviews with personnel.

and observation of

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license were conducted safely and in

accordance

with NRC requirements.

The team found that, in general.

engineering activities were performed in

accordance

with regulatory requirements.

The team observed,

however,

a lack

of a questioning attitude

on the part of your engineering staff and several

areas

requiring attention.

These

areas

included acceptance criteria for

testing, calculations,

and the relief valve program.

The resolutions to

problems that you identified often did not consider the generic aspects

of the

problem.

The team considered that, in general,

the engineering staff was

reacting to problems.

rather

than proactively identifying and resolving

problems.

Some performance

issues identified by the team had also been

identified by your self-assessment

activities but had not been fully

addressed.

Based

on the results of this inspections

certain licensed activities appeared

to be in violation of NRC requirements.

as specified in the enclosed

Notice of

Violation (Notice).

Violation A is of'oncern

because it showed weakness

in

implementing procedure

requirements.

Violation B is of concern

because it

demonstrated

weak technical. support for the acceptance criteria of some

procedures.

Violation C is of concern

because it showed

a weak understanding

of pressure relief valves.

In addition, the failure to maintain

a current qualification package for

safety-related electrical

equipment in accordance

with 10 CFR 50.49(d)

was

identified as

a violation.

Also, the failure to address

the generic issue of

an erroneous

drawing revision was identified as

a violation.

These two

violations are not being cited because

the criteria in paragraph VII.B.(2) of

Appendix

C to 10 CFR Part 2 of the

NRC "Rules of Practice,"

were satisfied.

Unresolved

items were identified concerning the ability of the standby service

water system to perform its safety-related

functions under design basis

temperatures:

the ability of the automatic depressurization

system check

valves to maintain the accumulators

pressurized

in order for the automatic

depressurization

system relief valves to remain operable;

and, the ability of

safety-related

pressure relief valves to perform thei r design functions

without knowing the status of the adjustment rings.

These matters

are

discussed

in Sections

2. 1.2. 1. 2.2.3. 1,

and 2.2.3.2,

respectively, of the

enclosed

inspection report.

You are required to respond to this letter and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should document the specific actions taken

and any additional

actions you plan to prevent recurrence.

Your response

may reference

or

include previous docketed

correspondence. if the correspondence

adequately

addresses

the required response.

After reviewing your response to this

Notice. including your proposed corrective actions

and the results of future

inspections,

the

NRC will determine whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requi rements.

In accordance

with 10 CFR 2.790 of the NRC's "Rules of Practice,"

a copy of

thi s letter, its enclosures,

and your response will be placed in the

NRC

Public Document

Room (PDR).

To the extent possible,

your response

should not

include any personal

privacy, proprietary. or safeguards

information so. that

it can

be placed in the

PDR without redaction.

However, if you find it

necessary

to include such information, you should clearly indicate the

specific information that you desire not to be placed 'in the

PDR,

and provide

the legal basis to support your request for withholding the information from

the public.

The responses

directed

by this letter

and the enclosed

Notice are not subject

to the clearance

procedures

of the Office of Management

and Budget

as required

by the Paperwork

Reduction Act of 1980,
