PROPOSED INPOSITION OF CIVIL PENALTY S50,000 This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 will be issued on or about Hay 18, 1995, to the Washington Public Power Supply System.
The action is based on violations which collectively represented a significant breakdown in the control of licensed activities.
The consequences of the breakdown contributed to violations of three Technical Specifications for control room emergency filtration operability.
The violations have been classified, in the aggregate, as a Severity Level III problem which has a base civil penalty of
$50,000.
After consideration of the escalation and mitigation factors, no adjustments were warranted.
,It;should -.be-noted.:that the licensee has not been specifically informed of the enforcement action.
The schedule of issuance and notification is:
Mailing of Notice Telephone Notification of Licensee The State of Washington will be notified.
Hay 18, 1995 Hay 18, 1995 The licensee has thirty days from the date of the Notice in which to respond.
Following NRC evaluation of the response, the civil penalty may be remitted, mitigated, or imposed by Order.
