REPORT 50-397/95-09

AND NOTICE OF VIOLATION

This refers to the inspection

conducted

by Nr.

R.

C. Barr,

and the inspectors

identified in this report, of this office on March

5 through April 22,

1995.

The inspection

included

a review of activities authorized for your Washington

Nuclear Project

2 facility.

At the conclusion of the inspection,

the findings

were discussed

with those

members of your staff identified in the enclosed

report.

Areas

examined during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection

consisted of selective

examina-

tions of procedures

and representative

records,

interviews with personnel,

and

, observations

by the inspectors.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The violation is of concern

because it resulted

in

operation of selected

containment

atmosphere

monitors in excess

of their

qualified life and indicates

a potential for other components

having

been

assigned

an inappropriate qualified life.

Our inspectors

observed

inconsistencies

in the control

room

operators'mplementation

of your management

expectations

for formality, command

and

control, communications,

and log keeping.

Control

room operators effectively

implemented expectations

in responding

to the reactor trip of April 5,

1995,

but control

room operators

did not consistently

implement your expectations

during the reactor

shutdown of April 21

and

22,

1995.

Your continued

attention to improving consistent

operator

implementation of your expectations

is encouraged.

Section

2.1 of the enclosed

inspection report discusses

the identification by

your staff of a failed strut in the nonsafety outlet piping from Feedwater

Heater

6B.

While initial corrective actions

were viewed

as prompt

and

appropriate, it was noted that engineering

had communicated

to operations

the

importance of minimizing time in hot standby to mitigate the potential for

thermal stratification conditions.

We suggest

that you review this matter

related to your operations

to assess

the potential

challenge this presents

to

operators

during plant startup or shutdown, particularly with regard to

feedwater control.

In addition,

you identified that measuring

and test equipment deficiency

reports

were not properly dispositioned.

Your investigation of this matter

identified that

a low level individual willfullyviolated approved

procedures.

This violation appeared

to be

an isolated action

and your actions to correct

this problem were timely and appropriate.

In accordance

with

Section VII.B.(2) of 10 CFR Part 2, Appendix C,

"General

Statement of Policy

and Procedure for NRC Enforcement Actions," this violation was not cited.

You are required to respond

to this letter- and should follow the instructions

specified

in the enclosed

Notice when preparing your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

Your response

may reference

or

include previous docketed

correspondence, if the correspondence

adequately

addresses

the required

response.

After reviewing your response

to this

Notice, including your proposed corrective actions

and the results of future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10

CFR 2.790(a) of the NRC's "Rules of Practice,"

a copy of

this letter

and its enclosures will be placed

in the

NRC Public Document

Room

(PDR).

To the extent possible,

your response

should not include any

personal

privacy, proprietary,

or safeguards

information so that it can

be

placed in the

PDR without redaction.

However, if you find it necessary

to

include such information, you should clearly indicate the specific information

that you desire not to be placed

in the

PDR and provide the legal

basis to

support your request for withholding the information from the public.

The responses

directed

by this letter

and the enclosed

Notice are not subject

to the clearance

procedures

of the Office of Management

and Budget

as required
