INSPECTION
REPORT 50-397/94-34
This refers to the inspection
conducted
by Hr.
S. J.
Campbell of this office
on January
23 through February
15,
1995.
The inspection
included
a review of
activities authorized for your Washington
Nuclear Project-2 facility.
At the
conclusion of the inspection,
the findings were discussed
with you and those
members of your staff identified in the enclosed
report.
These
inspection
items were discussed
with your staff by telephone
on February
15,
1995.

Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted
safely
and in
accordance
with NRC requirements.
Based
on the results of this inspection,
four apparent violations were
identified and are being considered for escalated
enforcement
action in
accordance
with the "General
Statement
of Policy and Procedure for NRC
Enforcement Actions" (Enforcement Policy),
Accordingly, no Notice of Violation is presently
being issued for these
inspection findings.
In addition,
please
be advised that the number
and
characterization
of apparent violations described
in the enclosed
inspection
report
may change
as
a result of further
NRC review.
These
apparent
violations are of particular concern
because
they appear to represent
a
breakdown
in activities associated
with the design,
maintenance,
testing,
and
operation of the control
room emergency filtration system
and the corrective
action program.
The first apparent violation is the failure to comply with a Technical
Specification action statement
for placing the unit in hot shutdown within the
allowed time because
an emergency filtration system
was inoperable.
This
apparent violation is of concern
because
the process
in identifying an
emergency filtration system
as
a result of a missing gasket
on Door
3 was inadequate
and,
as
a consequence,
a Technical Specification required
plant shutdown
was not conducted.

The second
apparent violation is the failure to comply with a Technical
Specification action statement for initiating and maintaining the control
room
ventilation system in the emergency
mode of operation while an emergency
filter unit was inoperable
in Conditions 4,
5,
and while moving irradiated
fuel.
This apparent violation is of concern
because
licensed
operators
missed
opportunities to evaluate
and identify the degraded
emergency filter unit as
being inoperable for an extended
period
as
a result of deluge water leaking
water onto the carbon filter.
As
a consequence,
the licensee failed to
initiate and maintain the operable
Train A emergency filtration system in the
pressurization
mode of operation with the Train
B emergency filter unit
inoperable while the plant was in operating Conditions 4,
5,
and while moving
irradiated fuel.
The third apparent violation is the failure to comply with a Technical
Specification action statement
for placing the unit in startup
and hot
shutdown within the allowed time because
both trains of the emergency
filtration system
were inoperable.
This apparent violation is of concern
because
licensed
operators failed to recognize that both trains of the
emergency filtration system were inoperable
as
a result of holes
opened
in the
control
room boundary
and therefore
a Technical Specification required plant
shutdown
was not conducted.
The fourth apparent violation involved
a failure of established
measures
to
ensure that conditions
adverse
to quality were promptly identified and
corrected
as required
by Criterion XVI of Appendix
8 to
This apparent violation involved multiple failures to effectively implement
corrective action processes
for conditions which rendered
Technical
Specification required
equipment
This violation is of concern
because
licensed
operators
and system engineers
missed
several
opportunities
to assess
the degraded
emergency filtration system
and identify that the
emergency filtration system
was inoperable.
An enforcement
conference
to discuss
these
apparent violations has
been
scheduled for April 7,
1995.
The decision to hold
an enforcement
conference
does
not mean that the
NRC has determined that
a violation has occurred or
that enforcement
action will be taken.
The purposes
of this conference
are to
discuss
the apparent violations, their causes
and safety significance;
to
provide you the opportunity to point out any errors in our inspection report;
and to provide
an opportunity for you to present
your proposed corrective
actions.
In particular,
we request that you address
in detail
the apparent
weaknesses
in utilization of corrective action processes
and weaknesses
in the
design modification process
and the interface with maintenance
and operations
in implementing modifications which we believe were significant contributors
to the apparent violations of Technical Specifications.
In addition, this is
an opportunity for you to provide any information concerning your perspectives
on:
(1) the severity of the violations;
(2) the application of the factors
'that the
NRC considers
when it determines
the
amount of a civil penalty that
may be assessed
in accordance
with Section VI.B.2 of the Enforcement
Policy;
and
(3) any other application of the Enforcement Policy to this case,

including the exercise of discretion in accordance
with Section VII.
You will
be advised
by separate
correspondence
of the results of our deliberations
on
this matter.
No response
regarding
these
apparent violations is required at
this time.

conference will be open to public observation
in accordance
with the Commission's trial program
as discussed
in the enclosed
Federal
~Re ister notice
(Enclosure 2).
Although not required,
we encourage
you to
provide your comments
on
how you believe holding this conference
open to
public observation
affected your presentation
and your communications with the
NRC.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosure will be placed in the
NRC Public Document
Room.
