This refers to the inspection
conducted
by Mr. D. L. Proulx,
and inspectors
identified in this report, of this office on September
19 through October 30,
1994.
The inspection
included
a review of activities authorized for your
Washington Nuclear Project-2 facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the
enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted
safely
and in
accordance
with NRC requirements.
During this inspection period,
a number of significant issues
arose
(including
several
identified by quality assurance)
that demonstrated
strong
management
and staff followup and corrective actions in response
to emergent
issues.
Your continued effort in this area is encouraged.
Based
on the results of this inspection,
a violation of NRC requirements
was
identified involving your failure to store
an overhead
crane
in
a seismically
safe location in accordance
with approved
drawings.
This violation is of
concern
because it is similar to
a previous violation identified in Inspection
Report 50-397/93-20.
You are required to respond to this letter and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
