(NOTICE OF VIOLATION)

This refers to the inspection

conducted

by Hr.

D. L. Proulx,

and the

inspectors

identified in this report, of this office on August

7 through

September

17,

1994.

This inspection

included

a review of activities

authorized for your Washington

Nuclear Project

2 facility.

At the conclusion

of the inspection,

the findings were discussed

with you and those

members of

your staff identified in the enclosed

report.

Areas

examined during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted of selective

examina-

tions of procedures

and representative

records,

interviews with personnel,

and

observations

by the inspectors.

We are pleased

that your staff achieved

a "blackboard" in the control

room on

September

15,

1994; e.g.,

no annunciators

were lighted.

We recognize that

significant resources

and

management

attention

were expended

to relieve the

operations staff of any work-arounds

and other deficiencies

to facilitate

saFety operation

oF the plant.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

This violation identified

a failure by your maintenance

organization to follow established

requirements

in that seismic tubing

supports

were not configured in accordance

with drawing requirements.

Although only one violation of NRC requirements

was identified,

NRC inspectors

identified that:

(1) the control

room service water flow

indication was less

than technical

specification requirements;

(2) on two

occasions

a breaker

on motor control center

7AA was closed without operations

department

knowledge;

(3) your staff did not fully understand

the reasons

for

having diesel

generator

fuel filter selector

switches

aligned to the "Both"

position;

(4) valve labels

and tubing supports

were missing from a modified

containment

atmosphere

control

(CAC) system transmitter installation;

(5)

completed

CAC transmitter modification test documentation

did not conform to

established

requirements,

despite

having

been

reviewed

by your staff;

(6)

calibration procedures

for the newly installed

CAC transmitter could not be

implemented

as written;

and '(7) there

was

a potential for a breach of

containment

by placing

a loss of coolant radiation monitor on line as required

by your procedure for evaluating

core damage,

among others identified in this

report.

Given these

issues, it is clear that

a more self-critical,

questioning attitude is sometimes

needed

by certain of your staff.

We

encourage

your actions to effect such

improvement.

You are required to respond to this letter

and should follow the .instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should

document the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed corrective actions

and the results of future

inspection,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10 CFR 2.790(a) of the

NRC's "Rules of Practice,"

a copy of

this letter

and its enclosures will be placed in the

NRC Public Document

Room.

The response

directed

by this letter and the accompanying

Notice are not

subject to the clearance

procedures

of the Office of Hanagement

and Budget

as

required

by the Paperwork Reduction Act of 1980,

Pub.

L.96-511.
