This refers to the inspection
conducted
by Mr.
R.
C. Barr,
and the inspectors
identified in the enclosed
report, of this office on April 2 through
Hay 14,
1994.
This inspection
included
a review of activities authorized for your
Washington Nuclear Project
2.
At the conclusion of the. inspection,
the
,findings were discussed
with you and those
members of your staff identified in
the enclosed report.
Areas
examined during this inspection
are described
in the enclosed
inspection
report,
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
These violations concerned
your failure to adhere to
procedures
for refueling
and for the control of compressed
gas cylinders
and
your failure to provide effective corrective actions for erratic recirculation
flow control valve operation.
The failure to restrain
compressed
gas
cylinders
was
a repeat violation that
NRC inspectors first identified in
NRC
Inspection
Report 50-397/93-50.
These violations reinforce the need for
Supply System
management
to continue efforts to assure
procedure
adherence
in
your future activities.
An additional violation concerned
the failure to
provide
an adequate
procedure for refueling operations
in that
an error in the
Nuclear
Component Transfer List allowed
a fuel bundle to be misoriented.
This
violation is not being cited because
the criteria of paragraph
VII.B of
Appendix
C to
10 CFR Part 2 of the
NRC's "Rules of Practice,"
were satisfied.
We are concerned
about the problems which you experienced
during the conduct
of refueling activities
and the circumstances
involved in the trip of the
These
problems resulted
from weaknesses
in your processes
that included:
(1) inadequate
communication
and coordination within and
between organizations,
(2) inadequate
independent
supervisory oversight
and
verification of activities,
and
(3) personnel
not adhering to procedures.

acknowledge your efforts to preclude
these- situations;
however,
we urge you to
continue those efforts with an
e'ven greater
sense of purpose
and self-critical
behavior.
You are required to respond to this letter
and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspection,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a) of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the
NRC Public Document
Room.

directed, by this letter and the accompanying

subject to the clearance
