This refers to the inspection
conducted
by Hr.
R.
C. Barr,
and the inspectors
identified in the enclosed report, of this office on January ll through
February
21,
1994.
This inspection
included
a review of activities authorized
for your Washington Nuclear Project
No. 2.
At the conclusion of the
inspection,
the findings were discussed
with you and those
members of your
staff identified in the enclosed report.
Areas examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Attachment
2 to the enclosed report also
addresses
the management
meeting held in our office on February 4,
1994.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
These violations concerned
your failure (1) to implement
and maintain
an alarm response
procedure prescribing
adequate
immediate
and
long-range actions,
(2) to declare
Average
Power
Range Monitor (APRM)
Channel
when performing maintenance
that rendered it incapable of
performing its safety functions,
and (3) to perform required time response
testing for certain relays
and contacts
in low pressure
emergency
core cooling
system actuation circuitry.
The inspection
also noted
a non-cited, violation
(NCV) involving the failure to test actuation logic relays
and contacts for
certain safety-related
pumps
and valves.
The inspection
also identified several
examples of weaknesses
in operator
alertness,
shift turnover,
and the control of work.
We are concerned
about
these
weaknesses
because
they indicate
a need for continued strengthening
of
management
oversight to sustain
performance
improvements
in these
areas.
You are required to respond to this letter
and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future

NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
