REPORT
NO. 50-397/93-45)
This refers to the inspection
conducted
by Messrs.
R.
C. Barr,
D. L. Proulx,
S.
P.
Sanchez,
G.
L. Johnston,
and D. L. Solorio of this office on October
19
through
November 29,
1993, of activities authorized
by
NRC Operating
License
No. NPF-21,
and to the discussions
of findings held by the .inspectors
with Hr.
J. Swailes
and other members of the Supply System staff at the conclusion of
the inspection.
This also refers to the management
meeting held in the
Region
V office in Walnut Creek, California,
on November 2,
1993.
A summary
of the discussions
held during this meeting
and
a copy of materials
presented
by the 'Supply System
are included in the enclosed
inspection report.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examina-
tions of procedures
and representative
records,
inter views with personnel,
and
observations
by the inspectors.
We noted during the inspection that your Technical Specifications
Surveillance
Improvement
Program continues to identify and resolve several
meaningful
and
safety significant technical
issues.
We are encouraged
by the meaningful
results of this initiative and
commend you for your staff's personal
efforts
and overall contribution of this initiative to improved plant safety.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Vi'olation (Notice).
These violations con-
cerned:
(1) three
examples of failure to follow surveillance
and annunciator
procedures;
(2) the failure to follow posted radiation protection instruc-
tions;
and (3) the improper erection of scaffolding installations
near safety-
related
equipment.
The inspection
also noted four non-cited violations
(NCVs)
involving: (1) the failure of the Nuclear Safety Assurance
Division (NSAD) to
perform documented
reviews of WNP-2 LERs;
(2) the failure to initiate
a
required fire impairment;
(3) the failure to test isolation logic relays;
and {4) the failure to develop
an adequate
procedure for locked valves.
We are concerned
about the violation associated
with the failure to enter the
Technical Specification
(TS) Limiting Conditions
For Operation

Statements
for equipment which is rendered
inoperable during the performance
of surveillance testing.
This issue
has
been previously addressed
in Generic Letter 91-18, which clearly indicated that safety equipment which is removed
from service for TS surveillances
and rendered
incapable of performing its
safety functions is inoperable.
The failure to identify and incorporate this
position into plant procedures
in a timely manner is considered
to be
a
further example of a weakness
in reviewing industry experience,
communi-
cations,'nd
teamwork.
We also noted that Operations Division personnel
were
responsible for three of the procedure violations because
your staff. were
either not aware of the procedural
requirement 'or incorrectly believed they
had met the procedural
requirements.
Collectively, these
instances
indicate
a
continuing
need for strong
management
oversight to ensure
the proper
implementation of plant procedures.
As discussed
in Paragraph
4 of the enclosed
inspection report,
the inspection
also identified that system engineers
(SEs)
were not consistently
meeting your
expectations
regarding the performance of weekly, monthly,
and quarterly
system walkdowns.
We note that the Supply System defined its intention to
perform these
SE walkdowns in response
to a Notice of Violation (NOV) issued
with NRC Inspection
Report No. 50-397/91-24.
The failure to successfully
implement this program is of concern
because
our inspection identified several
long-standing deficiencies that could have
been identified by your SEs during
their walkdowns.
Nevertheless,
we recognize that you have subsequently
initiated actions to properly implement this program.
You are required to respond to 'this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document
the specific actions taken
and any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosures,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the Paperwork Reduction Act of 1980,
Pub.
L.96-511.
