PROPOSED IMPOSITION OF CIVIL PENALTY $75,000 This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of
$75,000 will be issued on or about November 10, 1993 to the Washington Public Power Supply System.
This action is based on the failure to follow procedural requirements with regard to simultaneously placing both trains of the residual heat removal system in the suppression pool cooling mode while the plant was in operation.
The three separate occasions that this occurred have collectively been classified as a Severity Level III violation.
The base civil penalty was escalated by 50 percent after partially offsetting escalation for prior poor performance and corrective actions with mitigation for licensee identification.
It should be noted that the licensee has not been specifically informed of the enforcement action.
The schedule of issuance and notification is:
Mailing of Notice November 10, 1993 Telephone Notification of Licensee November 10, 1993 The State of Washington will be notified.
The licensee has thirty days from the date of the Notice in which to respond.
Following NRC evaluation of the response, the civil penalty may be remitted, mitigated, or imposed by Order.
