conducted

through September
6,
1993, of activities authorized
by
NRC Operating
License
No. NPF-21,
and to the discussions
of findings held by the inspectors
with Hr.
J. Swailes
and other members of the Supply System staff at'he
conclusion of
the inspection;
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
The cited violation
involved two examples of personnel
not following procedures.
In the first
example,
craftsmen did not stop to obtain
an approved deviation for a
procedure that could not be strictly followed,
and in the second
example,
craftsmen did not initiate a problem evaluation request
(PER) after observing
the incorrect assembly of a valve.
In addition, five non-cited violations
were noted.
These involved two instances
of inadequate
post-maintenance
testing,
two instances
of procedure
nonadherence
caused
by the lack of self-
checking
by I&C technicians,
and
an inadequate
surveillance
procedure.
In addition, this inspection identified three instances
in which it appeared
that Licensee
Event Reports
(LERs) did not contain
a full and clear discussion
of the events,
or when
a documented
analysis
could not be made available to
the inspector to support assertions
made in the
LERs.
We will continue our
reviews of these
LERs following your further review.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and any additional

actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspection,
the
NRC will determine whether further
NRC enforcement
action is
necessary

compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosure,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Hanagement
and Budget
as
required
by the Paperwork Reduction Act of 1980,
Pub.
L.96-511.
