NOTICE OF VIOLATION (NRC INSPECTION
REPORT
NO. 50-397/93-20)
This refers to the special
inspection
conducted
by Messrs.
R.
C. Barr and
D. L. Proulx of this office on April 2 through
Hay 17,
1993, at the Washington
Nuclear Project
Number
2 (WNP-2).
The results of this inspection
were
documented in the referenced
NRC inspection report,
which was transmitted to
you on June
1,
1993.
These
issues
were discussed
with you during an
enforcement
conference
held in the Region
V Office on June
29,
1992.
The
enforcement
conference
was
summarized in Meeting Report
No. 50-397/93-27,
transmitted to you on July 9,
1993.
The enclosed
Notice of Violation (Notice) refers to a March 31,
1993, contain-
ment atmospheric
control
(CAC) system test that exceeded
the Technical
Speci-
fications
(TS) limit for drywell-to-suppression
chamber
bypass
leakage.
This
event
was identified by a licensed operator
and
a system engineer during the
performance of the test
and was documented in your Licensee
Event Report
(LER)
No. 50-397/93-16,
dated April 30,
1993.
This was caused
by your failure to
take appropriate corrective actions to preclude repetition of a condition
adverse to quality.
On March 30,
1992, the
NRC had issued
a Notice of
Violation after the resident inspector identified that the Supply System
had
reviewed
and approved
a test of the
CAC system which, if implemented,
would
have resulted in drywell-to-suppression
chamber
bypass
leakage greater
than
allowed by the TS.
On March 31,
1993, while WNP,-2 was at full power, the
Supply System performed
a similar test that established
a drywell-to-
suppression
chamber
bypass greater
than allowed by the TS.
Although the enclosed violation was of limited safety significance,
we
strongly considered
issuing
a Severity Level III violation to emphasize
the
added significance of the situation
based
on the Supply System's failure to
implement effective corrective actions for the March 30,
1992, violation which
involved a similar root cause.
To improve overall performance
we consider it
essential
for the Supply System to assure that corrective actions
are
effective.
In this case,
however,
we recognize that
a licensed operator
and
a
system engineer identified that the
TS limit for suppression
chamber
bypass
leakage
had been
exceeded
while testing
was in progress.
The identification
of the bypass
leakage
demonstrated
a questioning attitude
on the part of the
operator,
a trait which we strongly encourage.

This event
was also properly reported in LER 50-397/93-16,
and the Supply
System's
followup review identified the failure to take effective corrective
actions for the March 30,
1992, Notice of Violation as
a primary cause for
this violation.
Me also note that your actions in response
to the Narch 31,
1993 violation (as discussed
in LER 50-397/93-16
and at the June 29,
1993,
enforcement
conference)
have
been aggressive.
These actions indicated your
understanding that the Narch 31,
1993,
event resulted
from a failure to ensure
that adequate
corrective actions
were taken following a previous violation.
You have also taken steps to assure that the corrective action program is
improved.
In addition, you have initiated actions to improve the procedure
verification and validation process
and plan to continue to improve the TS
surveillance
program.
Accordingly, we have decided to cite this violation at
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice of Violation when preparing your response.
In your response,
you should document the specific actions
taken
and any
additional actions
you plan to prevent recurrence.
After reviewing your
response
to this Notice, including your proposed corrective actions
and the
results of future inspections,
the
NRC will determine whether further
NRC
enforcement
action is necessary
to ensure
compliance with NRC regulatory
requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter, its enclosure,
and your response will be placed in the
NRC Public
Document
Room.
The responses
directed
by this letter and the enclosed Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork Reduction Act of 1980,
