This refers to the inspection
conducted
by Messrs.
R. C. Barr,
D. L. Proulx,
and
W. L. Johnson of this office on March 30 through
May 10,
1993, of
activities authorized
by
NRC Operating
License
No. NPF-21,
and to the
discussions
of findings held by the inspectors with Mr. J.
Baker and other
members of the Supply System staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears- that certain of your
activities were not conducted in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice) .
One violation consists of
several
examples of personnel
not following procedures.
Although these
examples
represented
administrative deficiencies,
we are concerned that
procedure
adherence
problems continue to occur
and that your corrective
actions for similar problems identi fied by the
NRC during the past
two years
have not been effective in preventing their recurrence';
As indicated in your
recent
response
to SALP Report No. 93-04, additional
management
attention in
this area
appears
warranted.
The second violation concerns
not maintaining
the plant's
drawings current with respect
to Thermolag configurations.
This
violation is significant because it uncovered
a potential deficiency in your
response
to
NRC Generic Letter 92-08, in that you would have failed to perform
cabling ampaci ty derating calculations for all the cabling protected
by
Thermolag
as previously committed.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspection,
the
NRC will determine
whether further
NRC enforcement
action is

compliance with NRC regulatory requirements.
