at WNP-2.

meeting

by your staff at the meeting are also enclosed.

During this meeting,

we discussed

the events

during the period from January

21

through February

16',

1993,

and weaknesses

which were highlighted by these

events.

In particular',

we pointed out that your staff did not appear to be

performing as

a single team, with Operations

in the lead

and other organiza-

tions clearly supporting Operations

to assure

consistent

improvement in plant

performance.

We noted that poor plant material condition, poor procedure

compliance,

weak personnel

performance

including the lack of a questioning or

intrusive attitude,

and insufficient in-plant supervisory attention

had

contributed significantly to the problems you encountered

during this period.

Additional oversight of these

concerns

by your senior

management

i s strongly

encouraged.

In accordance

with 10 CFR 2.790 of the NRC's "Rules of Practice,"
