This refers to the inspection
conducted
by Hessrs.
W.
P. Ang,
R.
C. Barr and
D. L. Proulx of this office on November
16 through
December
27,
1992 of the
activities authorized
by
NRC Operating
License
No. NPF-21,
and to the
discussions
of findings held by the inspectors with Hr. J.
Baker
and other
members of the Supply System staff following the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these areas,,the
inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
This violation involved
two examples of not adequately
maintaining control
room logs with respect
to
potentially reportable
events.
The violation suggests'
weakness
in your
operators'iagnosis
and logging of plant activities.
Although the violation
has
been categorized
as
a Severity Level V, the enclosed
Notice is being
issued
because
the operators'ailure
to document the isolation significantly
contributed to the Supply System's
reporting the event approximately three
days. after the event occurred
instead of the required four hours.
Paragraph
5 discusses
the difficulties you have encountered
in the development
of your Reliability Centered
Haintenance
(RCH) program.
While we recognize
that the development of this program is
a noteworthy initiative on your part,
your delay in challenging your contractor's
slow progress
indicates insuffi-
cient management
oversight of this effort.
You are required to respond to this letter
and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future

inspection,
the
NRC will decide whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter,
the enclosures,

your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter
and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Hanagement
and Budget
as
required
by the Paperwork Reduction Act of 1980,
Pub.
L.96-511.
