This refers to the inspection
conducted
by Messrs.
W. P. Ang, D. L. Proulx and
K. E. Johnston of this office on October
5 through November
15,
1992 of
activities authorized
by
NRC Operating
License
No. NPF-21,
and to the
discussions
of findings held by the inspectors with Mr. J.
Baker and other
members of the Supply System staff at the conclusion of the inspection.
Areas examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
These violations indicate
weaknesses
in your corrective action program,
your Clearance
Order process,
and your plant painting program.
The resident inspector
found
a radiation
area
boundary sign down, the fourth posting problem identified by the resident
staff this year.
While these posting problems did not result in an overexpo-
sure
and were not individually of major significance,
they indicate that your
corrective actions for previous violations have not been fully effective.
In addition,
an operator
hung
a Danger Tag despite
a discrepancy
involving the
labels affixed to the electrical cubicle.
This cubicle had
been mislabeled
during painting activities.
Although in this case
no personal
injury, equip-
ment damage,
or operability concerns
resulted,
this violation is of particular
concern
because
deficiencies
in your painting program could result in similar
occurrences
with increased
consequences.
Your attention is also invited to Paragraph
10 of the enclosed report, which
discusses
a review of your corrective action tracking program.
While this
inspection did not identify any specific violations or deviations resulting
from poor performance of the program,
the findings indicate
a need for
improvement in your tracking of NRC commitments
and other actions.

You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions taken
and any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine whether further
NRC enforcement action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosures,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the Paperwork Reduction Act of 1980,
Pub.
L.96-511.
