we discussed
the August 15,
1992 core instability event
and the, weaknesses
in your management
controls
and oversight which permitted
the event to occur.
In particular,
inadequate
procedures,
insufficient
response
to industry experience,
and weakness
in the review of core design all
contributed significantly to this event.
Close attention to these. issues
by
your senior sanageaent
is strongly encouraged.
