This refers to
a special test
team inspection
conducted
at the Washington
Nuclear Project
No.
2 by Hr.
P.
P. Narbut and
a team of NRC personnel,
from
July
13 through August 14,
1992.
At the conclusion of the inspection,
the
findings were discussed
with those
members of your staff identified in the
enclosed report.
Areas
examined during this inspection
were post maintenance
testing,
post
modification testing,
technical
specification surveillance testing,
ASHE
inservice testing,
and predictive maintenance
testing.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and records,
interviews with personnel,
and observations of activities by the inspectors.
The testing
areas
examined
appeared
to be performed in a manner which provides
generally
adequate
assurance
of system operability.
However,
a number of
weaknesses
in your performance of testing activities were identified.
Specifically:
~
Personnel
conducting tests did not always follow the test procedural
requirements,
nor did they annotate
why they deviated
from the
requirements.
~
A number of test procedures
contained technical
or administrative
errors;
personnel
using these
procedures
did not always stop work when
an error was noted,
but rather
attempted
to use the inadequate
procedure.
~
Several
test procedures
did not adequately verify the acceptability of
the components
being tested.
In one case,
a replaced
switch was not
tested
before being placed in service.
The switch was important,
in
that it was used to transfer control of the Reactor
Core Isolation
Cooling System
(RCIC) from the control
room to the remote
shutdown
panel.

Several
test procedures
did not adequately verify the acceptability of
the components
being tested.
In one case,

tested

in service.
The switch was important,
in
that it was
used to transfer control of the Reactor
Core Isolation
Cooling System

room to the remote
shutdown
panel.
It was disappointing that the team found numerous
examples of failure to
follow procedures
given the attention this subject
has received at your
facility. Specifically,
many previous
examples of failures to follow
procedures
were brought to your attention
by the Electrical Distribution
System Functional
Inspection
(EDSFI) team inspection,
which was completed in
February
1992
(NRC Inspection
Report 50-397/92-01).
You identified
a
corrective action program for improving procedure
adherence
at the enforcement
conference
held in Walnut Creek
on Hay 27, 1992.. In addition,
you described
short
and long term corrective actions in your July 10,
1992 response
to the
Notice of Violation in the
EDSFI inspection report.
Although none of the individual instances
identified were of great
significance,
the findings of the enclosed
report indicate that your short
term actions to assure
procedural
adherence
have not been fully effective.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
You .are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and
any additional
actions you plan to prevent recurrence.
You should include, in your response,
short term actions that will make clear to your personnel
your policy
regarding procedural
adherence.
After reviewing your response
to this Notice,
including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed in the
NRC Public Document
Room.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Hanagement
and Budget
as required
by the Paperwork
Reduction Act of 1980,
