This refers to the inspection
conducted
by Messrs.
R.
C. Sorensen
and
D. L. Proulx of this office on July
13 through August 23,
1992 of activities
authorized
by
NRC Operating
License
No. NPF-21,
and to the discussions
of
findings held by the inspectors
with Hr. J.
Baker and other members of the
Supply System staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examina-
tions of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
This violation indicates
a weakness
in your ability to maintain proper posting for radiation areas.
Paragraph
4 of the enclosed
report discusses
your actions in response
to two
reactor
which resulted
from failures of safety/relief valve 3B.
While
we were concerned
that your cause
assessment
following the first failure was
not sufficiently thorough,
we wish to recognize
the extensive
root cause
assessment,
confirmatory testing,
and corrective actions which you undertook
following the
second
reactor
on July 11,
1992.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
=
response,
you should
document the specific actions taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
- Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
