This refers to the inspection
conducted
by Messrs.
R.
C. Sorensen
and
D. L. Proulx of this office on January
24
March 8,
1992 of activities
authorized
by NRC Operating
License
No. NPF-21,
and to the discussion of
findings held by the inspectors
with Mr. J.
Baker and other members of the
Supply System's staff at the conclusion of the inspection.
Areas examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with
personnel
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
These violations involved
a weakness
in your safety evaluation -for a test to be performed
on the
containment
atmospheric
control system,
and the failure of your staff to
operate
the control rod drive system in accordance
with the Technical.
Specifications
(TS).
The violation concerning
the special test of the containment
atmospheric
control
(CAC) system involved approval
by the Plant Operations
Committee
(POC)
of the performance of a special test which would have violated the require-
ments of your Technical Specifications.
Furthermore,
members of your staff
indicated that the test would have
been performed
had it not been questioned
by the
NRC resident
inspector.
This issue indicates that the safety evalua-
tion for this test
was too narrowly focused
on operability of the
CAC system,
without adequate
consideration of its possible
impact on other safety
features.
Citation of this violation in the enclosed
Notice is intended to
emphasize
the NRC's expectation that
POC reviews appropriately
address
such
concerns.
Although your staff identified the
TS violation concerning
improper operation
of the control rod drive system, it is being cited because
other criteria for
a non-cited violation,
as stated
in paragraph
V.G. 1 of 10 CFR Part 2, Appen-
dix C, were not satisfied.
In particular,
a Problem Evaluation

was not initiated
as required to provide for appropriate corrective actions
and assessment
of reportability,
and the issue
would likely not have
been
reported without the inspector's
intervention.
You, are required to respond to this letter and should .follow the instructions
specified in the enclosed
Notice when preparing your response.
In your-
response,
you should document the specific actions taken
and any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosure,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the Paperwork Reduction Act of 1980,
PL 96-511.
