Basis for Continued Operation for WNP-2 Following Operational Evaluation Conducted on March 21,22 1991 On March 14,1991, the Supply System submitted to you a Basis for Continued Operation following the failure of our Licensed Operator Requalification Program.
On March 21st and 22nd, an Operational Evaluation of our four operating crews was conducted.
One of the four crews (Crew B) failed the evaluation.
In addition, three individuals failed during the evaluation process.
The Supply System has carefully evaluated the basis for continued operation with three qualified crews-until our scheduled shutdown for refueling the weekend of April 12,1991.
P The followingactions have been taken to ensure the continued safe operation ofWNP-2.
This document has been reviewed with Senior Supply System Management and has received Plant Operating Committee review and approval.
The three individuals that failed the evaluation have been removed from all licensed duties.
They will not be returned to licensed duties until they have successfully completed an NRC approved Requalification Examination, similar to the process anticipated for the previous requalification examination failures.

We have created a three-shift rotation comprised of licensed operators who have successfully demonstrated their abilities during the recent NRC Operational Evaluation, The crews willwork twelve hour shifts as outlined in Attachment 1.
We willcontinue to comply with the overtime requirements specified in the WNP-2 Plant Technical Specifications. The Reactor Operator replacing the Reactor Operator who failed on Crew E has been evaluated by the NRC as a qualified replacement.

We have provided additional support to the operating crews while we are in this operational condition. Specifically, as of March 23,1991, we will add an additional Senior Reactor Operator to the control room staff to provide for additional shift oversight during normal operation and for emergency flow chart support during emergency conditions. In addition, the on site STA described in our previous BCO willnow be assigned to the Control Room (as contrasted with our previous assignment of capable of responding to the control room in 10 minutes). This shift manning is depicted in Attachment 2.

A remediation program for the Control Room Supervisor on Crew E and the Shift Manager on Crew B willbe developed, submitted to the NRC and executed with.
final examination by March 29, 1991.
Our management over-sight of control room activities described in our previous BCO willbe expanded'to include the new elements presented here. It willalso be expanded to ensure back shift coverage of shift activities.
The Supply System is confident that it can maintain safe operation ofWNP-2 with these administrative controls until our upcoming refueling outage in three weeks.
Our three shifts are comprised of experienced operators who have completed recent team command and control training and extensive evaluation.
Our control room man'ning is being augmented by an additional Senior Reactor Operator for shift over-sight and accident management ifnecessary.
In addition, we have expanded the role of the Shift Technical Advisor by increasing his presence in the control room at all times.
Our management and Quality Assurance oversight of control room activities willcontinue. Ifduring these over-sight tours we determine that the capabilities of our control room staff are-jeopardized, we willtake immediate steps to shut down the unit.
The Supply System willcontinue to evaluate the root cause for these license failures.
We expect to maintain a high level of communication with your office as we conclude our evaluation.
