and to the

discussion of findings held by the inspectors

with, Hr. J.

Baker

and other

members of the Supply System staff at the conclusion of the inspection.

Areas

examined during this inspection

are described

in the enclosed

inspec-

tion report.

Within these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with

personnel

and observations

by the inspectors.

Based

on the results of this inspection, it appears

that certain of your

activities were not conducted

in full compliance with NRC requirements,

as

set forth in the Notice of Violation, enclosed

herewith

as Appendix A.

These

violations involved insufficient corrective actions in response

to the

. May 1990 diesel

generator

bearing failure,

and performance of a surveillance

test without establishing

the specific conditions prescribed

by the Technical,

Specifications.

You are required to respond to this letter and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should document the specific actions taken

and any additional

actions you plan to prevent recurrence.

The violation involving diesel

generator

bearing oil leakage

and monitoring

is of particular concern to us in view of the significant bearing failure and

fire which you experienced

in Hay 1990.

As discussed

in paragraph

4 of the

enclosed

inspection report,

our inspectors

identified

a low bearing oil level

condition associated

with the Division I Emergency Diesel Generator,

which

had existed since the previous

day and

was not corrected for approximately

two days after its initial discovery

by plant personnel.

Further examination

of this issue identified two previous occasions

on which oil was

added after

comparable

delays,

without appropriate tracking of oil additions to monitor

the rate of oil leakage.

Your staff also

had not established

appropriate

acceptance

criteria specifying the minimum bearing oil level required for the

diesel

generator to be considered

operable.

These observations

indicate that

your staff has not responded sufficiently to the lessons

lear'ned

from the Hay

1990 bearing failure and fire, particularly in view of the narrow range of

acceptable

bearing oil level.

In your response

to the above Notice, please

address

the actions you plan to take to effectively resolve this concern.

Based

on the results of this inspection, it also appears

that certain of your

activities did not fully meet'a

commitment

made to the

NRC,

as set forth in

the Notice of Deviation, enclosed

herewith

as Appendix B.

You are requested

to provide,a response to'his Notice, as stated in Appendix B, Notice of

Deviation.

In accordance

with 10 CFR 2.790(a),

a copy of this letter, the enclosures,

will be placed in the

NRC Public Document

Room.

The responses
