APPENDIX A NOTICE OF VIOLATION Washington Public Power Supply System P. 0. Box 968 Richland, Washington 99352 Docket-No. 50-397 License No. NPF-21 f
As a result of the inspection
'conducted during the period June 25-27,
- 1986, and in accordance with NRC Enforcement Policy 10 CFR, Part 2, Appendix C, the following violation was identified:
A.
Technical Specifications, Section 6.11.1, "Radiation Protection Program,"
states in part:
"Procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be
'pproved, maintained, and adhered to for all operations involving personnel radiation exposure."
Plant Procedures Manual (PPM) 10.1.20, "Vacuum Cleaner Control," Revision 0, dated April 4, 1986, requires:
(1) controlled vacuum cleaners shall be designated and tagged as either wet type or dry type; (2) radiation surveys are performed prior to issue, prior to return to storage; and (3)
"Vacuum cleaners shall be returned to the issue station when the job is complete or daily..." Additionally, Radiation Work Permits Nos.
2-86-00006 dated March 18, 1986 and 2-86-00173 dated April 19, 1986 requires radiation surveys of vacuum cleaners be performed during services (e.g.,
- emptying, bag and filter replacement operations).
PPM 11.2.13.1, "Direct Area Radiation and Contamination Surveys," Revision 2, dated November 26,
- 1984, and PPM 11.2.13.4, "Documentation of Radiation and Contamination Survey Results," requires that all specific surveys be documented or recorded on an axea survey map.
Contrary to the above:

There were ten vacuum cleaners at the facility on June 26, 1986, that were not designated and tagged as either wet type or dry type.

None of the vacuum cleaners
'issued between the period April 24,
- 1986, and June 23, 1986, were returned to the issue point daily.

Less than 5Z of the routine survey results taken of vacuum cleaners during the period of April 24 to June 19, 1986, were documented.
4.
A survey was not documented for vacuum cleaner 812 that was serviced on May 12, 1986.
This is a Severity Level IV Violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply, including for each violation:
(1) the reason for 'the violations if
