Dear Mr. Maciuska:
On April 7, 1999, the NRC administered a generic fundamentals examination (GFE) section of the written operator licensing examination to an employee of your facility. Enclosed with this letter are copies of both forms of the examination, including answer keys, the grading results for your facility, and a copy of the individual's answer sheet. One individual was administered the examination and passed the examination.
Please forward the results to the individual along with the a copy of the answer sheet.
A "P" in the column labled RESULTS indicates a passing grade of 80 per cent or better on the GFE. Those individuals having an "F" in the RESULTS column failed the examination.
