SYSTEMATICASSESSMENT OF LICENSEE PERFORMANCE (SALP) REPORT
NO. 50-244/98-99 FOR THE R. E. GINNA NUCLEAR POWER PLANT
Dear Dr. Mecredy:
This letter forwards the R. E. Ginna SALP report for the period of August 25, 1996 to
February 21, 1998 (Enclosure
1) ~ This assessment
was conducted using the revised SALP
process that was implemented by the Nuclear Regulatory Commission (NRC) on July 19,
1993.
In this process the NRC staff evaluates the performance of licensees
in four
functional areas: Operations, Maintenance,
Engineering, and Plant Support.
The Plant
Support area includes
Radiological Controls, Security, Emergency Preparedness,
Fire
Protection, Chemistry, and Housekeeping.
Overall, performance at the Ginna Station was generally good; improvements were made in
a number of areas to address previous weaknesses.
Conservative decision making was
evident this period.
This performance was exemplified by the main transformer outage in
November 1996, during which the station made progress
in identifying and repairing
equipment problems, some of which were longstanding.
The outage was followed by an
extended period of stable plant operation, good plant equipment performance, and
improved interfaces between plant organizations. Management oversight of motor operated
valve and service water system issues improved, but implementation of the programs
continued to be a challenge.
Lapses in oversight resulted in some poor radiological work
practices.
Some lapses were also noted with operator control of plant configuration.
In
addition, procedure quality and adherence
problems occurred throughout the SALP period,
particularly in the maintenance
area.
Operations was rated Category 2. Performance
in plant operations was normally very
good.
Operator response to plant transients,
and control of startups and shutdowns,
co'ntinued to be good.
During the period, management
involvement and oversight were
normally evident, and were reflected in sustained
safe and stable plant operations.
Steps
were taken to address problems with Technical Specification interpretations seen in the
previous period.
Operations communications and interactions with other site organizations
were generally good throughout the period.
However, some lapses in operators'ontrol of
work and plant configuration occurred.
Examples included the inadvertent draining of
reactor makeup water to the waste holdup tank and the inadvertent opening of a reactor
coolant system power-operated relief valve.

Maintenance was rated Category 2. During this period, maintenance
activities were
generally effective.
Improvements were made to address
some past weaknesses
in
maintenance
processes.
Development and implementation of an integrated work schedule
improved the overall quality and timeliness of maintenance work and yielded improvements
in communication and coordination among operations, maintenance,
and engineering
organizations.
Management oversight of maintenance
improved and contributed to more
prompt identification and resolution of equipment deficiencies.
Progress was made in
addressing
and reducing the maintenance
backlog, yet the backlog remained high at the
end of the SALP period.
Some instances of inadequate
procedure adherence
and difficulty
with implementing maintenance administrative requirements were experienced during the
SALP period.
Engineering was rated Category 2. The system engineering group brought an increased
presence of design and licensing experience onsite and increased the ability of the system
engineers to support resolution of day-to-day problems in the plant.
Engineering work was
generally well controlled, as illustrated by good engineering work products and support of
troubleshooting efforts.
Progress was made in resolving some prior performance issues
associated with the auxiliary feedwater and service water systems.
Engineering
performance was good regarding control of the Ginna Station design basis.
However,
performance in some engineering areas was mixed.
For example, completion of the motor
operated valve program and some longstanding unresolved service water items continued
to challenge engineering.
Also, problems were noted with the vendor manual program.
Performance
in Plant Support was rated Category 2. Radioactive waste processing and
transportation were effectively managed and continued to exhibit a high level of
performance.
Improving trends in radiation exposure and personnel contaminations were
observed.
Weaknesses
in radiological boundary controls were identified early in the period
but improvements were made through training and management oversight.
Fire protection
was generally very good.
Emergency planning continued to perform well. Housekeeping
in
the Auxiliary Building has improved; however, housekeeping
deficiencies in the
containment during the refueling outage were observed.
Performance
in the area of
security declined, with weaknesses
noted in the fitness for duty program and the
implementation of the Vehicle Barrier System.
Enclosure 2 provides the schedule and basis for the NRC inspections of your facility
planned for the next year,
We willinform you of any changes required to the attached
inspection schedule.
The routine resident inspection effort is not included in this schedule.
