Dear Mr. Vissing:
In accordance with GL 96-01, the RGiIcE Nuclear Operations Group reviewed safety related logic circuits and Technical Specifications Surveillance Requirements (SRs) to ensure that all portions of safety related logic circuitry are adequately tested and that all SRs are formalized in plant procedures.
The GL 96-01 Review identified 16 cases (deficiencies) where untested logic components could have failed in an undetected manner and adversely affected a required safety function.
Those were subsequently tested and found to be operational with no failures.
These actions were reported to the NRC in.LER 96-005 dated June 17, 1996.
To date, no additional deficiencies have been identified, and all previously identified untested logic components are now permanently incorporated into plant surveillance procedures.
