SUBJECT:

NRC INTEGRATED INSPECTION REPORT 50-244/96-06 AND NOTICE

OF VIOLATION

Dear Dr. Mecredy:

From June 16 to July 20, 1996, the NRC completed

an inspection at your Ginna reactor

facility. In-depth program reviews and reactive inspection efforts occurred during the

inspection period.

The enclosed report presents the results of the inspection.

During the five week period covered by the inspection, the plant was operated safely.

Your staff's decision to shut down the plant on July 3, 1996, for several component

repairs was timely. Operators responded

well to several equipment malfunctions during

the shutdown process

and during the subsequent

return to power operations.

Good

operator actions were also noted during a simulator training exercise,

and good actions

were observed during mechanical maintenance

activities to remove a service water pump

and motor.

We observed

an effective and well implemented corrective action program.

We reviewed selected electrical modifications and noted they were of good quality and

well-supported with sound design basis technical information.

Your engineering staff made

a sound decision to enhance the reliability of an offsite power supply.

Also, we noted you

had in place a very effective program for the processing

of solid radwaste

and for the

shipping and storage of radioactive material.

However, based on the results of this inspection, the NRC determined that two violations

of NRC requirements

occurred.

These violations are cited in the enclosed

Notice of

Violation (Notice) and the circumstances

surrounding

each are described

in detail in the

attached inspection report.

One violation was cited for failure to lock eight service water

valves, as required by the Improved Technical Specifications

(ITS). This issue is of

concern because

it was found by the NRC and reflects an insufficient plant staff

knowledge of the new requirements

in the ITS.

The second violation concerned the failure to perform a safety evaluation regarding

a May

1994 modification of the auxiliary building service water system isolation valve.

This

violation is significant in that the 1994 modification made a four fold increase

in the stroke

time of this valve without a safety evaluation, and without evaluating the effect of this

change upon containment fan cooling or emergency

diesel generator analyses.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response.

In your response,

you should document

the specific actions taken and include any additional actions you plan'to prevent

Your response

may reference or include previous docketed correspondence,

if

the correspondence

adequately

addresses

the required response.

After reviewing your

response

to each Notice, including your proposed

corrective actions and the results of

future inspections,

the NRC willdetermine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

We are also concerned

about the incomplete status of the initial service water heat

exchanger thermal performance test program, which you committed to perform in response

to Generic Letter 89-13, "Service Water System Problems Affecting Safety-Related

Equipment."

For the 18 sets of service water heat exchanger thermal performance tests

performed, only the first (baseline) test of the diesel generator coolers had been fully,.

evaluated with an engineering report issued.

While your staff expected to complete the

review of these tests in 1996 (as discussed

during the inspection), we request that you

inform us regarding your specific plans to complete the reviews of all past service water

heat exchanger thermal performance tests.

Also, we request that you inform us regarding

your specific plans to complete the initial service water heat exchanger thermal

performance test program, including any additional thermal performance tests that you

deem necessary.

We request that you respond to this request within 60 days of receipt of

this letter.

In accordance

with 10 CFR 2.790 of the NRC's "Rules of Practice,"

a copy of this letter,

its enclosure(s),

and your response

will be placed int he NRC Public Document Room

(PDR). To the extent possible, your response

should not include any personal privacy,

proprietary, or safeguards

information so that it can be placed in the PDR without

redaction.

However, if you find it necessary

to include such information, you should

clearly indicate the specific information that you desire not to be placed in the PDR, and

provide the specific information that you desire not to be placed in the PDR, and provide

the legal basis to support your request for withholding the information from the public.

The responses

directed by this letter and the enclosed

Notice are not subject to the

clearance procedures

of the Office of Management

and Budget as required by the

Paperwork Reduction Act of 1980, Pub. L No. 96.511.
