NRC INTEGRATED INSPECTION REPORT 50-244/96-05 AND
'ear
Dr. Mecredy:
From May 5 through June
15, 1996, the NRC conducted
an inspection at your Ginna
reactor facility. The enclosed report presents the results of that inspection.
During most
of the six week period covered by this inspection, the plant was shutdown for the refueling
and steam generator replacement
outage.
Overall, we found that activities conducted
at
the Ginna station were performed in a safe manner with a high level of attention
maintained toward the health and safety of the public.
During this inspection, your staff successfully completed
a refueling outage within the
planned outage schedule,
and accomplished the complete replacement of both steam
generators without major technical problems.
During the outage, your staff displayed
excellent coordination executing
a well-prepared plan to restore the reactor containment
building to its original configuration and leak tightness,
and to assure its structural
integrity. The classroom
and simulator training to prepare operators for the subsequent
startup were well conducted
and effective.
The operations department performed an
effective review of procedures to identify changes
required because
of the new steam
generators.
Overall, your staff conducted the reactor startup and power ascension
in a
safe and controlled manner.
However, one instance occurred during startup of the plant
where the control room staff was not sufficiently cognizant of procedure or plant
configuration requirements
concerning the automatic start feature of the motor driven
auxiliary feedwater pumps.
This issue was left unresolved
pending further NRC review.
Based on the results of this inspection, the NRC has determined that a violation of NRC
requirements
occurred concerning the failure to establish proper containment closure prior
to refueling operations.
This violation is cited in the enclosed
Notice of Violation (Notice),
and the circumstances
surrounding the violation are described
in detail in the enclosed
report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed
Notice when preparing your response.
The NRC will
use your response,
in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of this letter
and its enclosures will be placed in the NRC Public Document Room.
