NRC
INTEGRATED INSPECTION
REPORT 50-244/96-03
AND
Dear Dr. Mecredy:

On May 4,
1996, the
NRC completed
an inspection at your Ginna reactor
facility.
The enclosed
report presents
the results of that inspection.
During most of the six week period covered
by this inspection,
the plant was
shutdown for the refueling
and steam generator
replacement
outage.
Overall,
we found that activities conducted
at the Ginna station were
performed in a safe
manner with a high level of attention maintained
toward
the personal
safety of outage workers
and toward the health
and safety of the
public.
The identification by
RG&E project management
of an unfavorable trend
in procedure
compliance
by the steam generator
replacement
project
(SGRP)
contractor,
and its resulting root cause
analysis
and corrective actions,
was
considered
to be
a positive
SGRP management
action.
We concluded radiological safety performance
during the
1996
and
refueling outage
was generally excellent.
Specifically,
we noted there
was
good work planning that resulted in minimized radiological
hazards,
and very
effective radiation protection staff coverage
and oversight of work
activities.
However, there were two violations of NRC requirements
identified that
involved an inadequate
survey to support the release
of potentially
contaminated
materials
from the station,
and inadequate
support for a change
to the updated
Final Safety Analysis Report relative to the use
and
application of continuous air monitoring equipment.
These violations are
cited in the enclosed
Notice of Violation (Notice),
and the circumstances
surrounding the violations are described
in detail in the enclosed
report.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your .response.
In your
response,
you should document the specific actions taken
and
any additional
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous docketed
correspondence, if the correspondence
adequately
addresses
the required response.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
