Dear Mr. Cooper:
We acknowledge receipt of the Region I Plant Performance Review (PPR) for Ginna
- Station, dated October 20, 1995.
We are pleased that notation was made of the generally good performance of the plant and our personnel during the PPR time period.
We do, however, believe that comments regarding corrective action thresholds and Radiation Work Permit compliance weaknesses warrant response.
We agree that ACTION reports could have been generated for the noted occurrences.
During the initial few months of operating under the new Nuclear Interface Procedure, "Abnormal Condition Tracking Initiation or Notification (Action) Report,"
(IP-CAP-1), all ACTION reports were reviewed by a single entity (the Operating Experience Group) so as to make a judgment on the level of uniformity in ACTION report generation.
Based on this effort, recommendations were made to modify the proce-dure, including an extensive list of examples to be used as guidance on the expected threshold for initiation of an ACTION report.
Revision 2
to IP-CAP-1 was issued on October 24, 1995.
Relative to human performance deficiencies regarding the wearing of secondary alarming personnel dosimetry when entering radiologically controlled areas, actions by our plant manager as noted in his October 24 memo to all personnel have been initiated to prevent further such occurrences.
