Dear Hr. Donnelly:
This refers to the routine safety inspection
conducted
by Hessrs.
R.
Leemon,
L. Olshan,
E. Plettner,
H. Phillips, T. Tongue,
and
R. Twigg of this office
from Hay 26 through July 15,
1993.
The inspection
included
a review of
activities at the Big Rock Point Nuclear facility authorized
by NRC Operating
License
No. DPR-6.
At the conclusion of the inspection,
the findings were
discussed
with those
members of your staff identified in the enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of a selective
examination of procedures
and representative
records,
observations,
and interviews with personnel.
Two significant concerns
were noted during the inspection period.
The first
concern
was in the area of general
housekeeping
in the containment
sphere.
Fibrous
and other debris
had accumulated
in sufficient quantity to potentially
affect the operability of the strainers
under accident conditions.
This is of
particular concern
because
even
though this condition
had
been brought to the
attention of shift supervision
on June
28,
1993,
the condition
had not been
corrected
by June
29,
1993,
when
NRC regional
management
toured the
same
areas.
We note that the loose material
was subsequently
removed,
and a,
control boundary
was established
around the reactor cavity after this
condition was brought to the attention of plant management
by regional
management.
The second
concern dealt with the conduct of shift briefings
during the outage.
During one briefing, numerous interruptions related to
outage
work were observed.
These interruptions
brought into question
the
effectiveness
of the briefing.
Certain of your activities appeared
to be in violation of NRC requirements,
as
specified in the enclosed
Notice of Violation (Notice).
The violation
involved failure to follow procedures
for control of combustibles
and for
reactor deck tool control
and housekeeping.
The violation is of concern
because
these failures to follow procedure
could result in
a fire affecting
safety related
equipment or foreign material entering the reactor cavity and
affecting the fuel.
We note that your staff had
been
made
aware of the
foreign material
concern
before the reactor
head
had
been
removed,
thus
providing an opportunity for you to correct the problem before it became
a
violation.
An additional violation involving the use of caution tags
was also

identified; however,
since the requirements
.Section VII.B were met, this violation is non-cited.
A written response
is required
and you should follow the instructions
specified in the enclosed
Notice.
In your response,
you should document the
specific actions
taken
and
any additional actions
you plan to prevent
recurrence.
After reviewing your response
to this Notice, including your
proposed corrective actions
and the results of future inspections,
the
NRC
will determine
whether further
NRC enforcement
action is necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the Commission's regulations,
a copy of
this letter, the enclosures,
and your response
to this letter will be placed
in the
NRC Public Document
Room.
The responses
directed
by this letter
and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the Paperwork Reduction Act of 1980,
PL 96-511.

any questions
you have concerning this inspection.
