This letter forwards the

R.

E. Ginna,

SALP report for the period September

12,

1993 to Narch ll, 1995.

This SALP was conducted

under the revised

SALP

process that was implemented

by the Nuclear Regulatory Commission

(NRC) on

July 19,

1993.

The revised

SALP process

rates

licensees

in four functional

areas:

Operations,

Naintenance,

Engineering,

and Plant Support.

The Plant

Support area includes:

Radiological Controls, Security,

Emergency

Preparedness,

Fire Protection,

Chemistry,

and Housekeeping

Controls.

Overall, the

NRC continued to see very good safety performance

at Ginna.

Nanagement

demonstrated

a proper safety perspective

and exhibited

improved

oversight

and involvement in plant activities.

The

RG&E safety review

committees strongly contributed to the conservative

operating philosophy

and

scope of corrective actions.

We found your self-assessment

activities to be

proactive, especially your recent efforts to review the operational

performance of the service water system:

With the exception of some lapses

in

the maintenance

area,

we found your reviews

and root cause

determinations

for

plant problems to be thorough

and comprehensive.

The numerous

examples of

maintenance

related

equipment installation or reassembly

problems, especially

during outage periods,

warrant

a broad review to identify root causes

and to

devise measures

for earlier identification of possible

adverse

performance

trends.

We noted improved performance in operations

and this area

was rated Category

1.

Operations

personnel

continued their strong performance

in operating the

plant and responding to events while improvement

was observed

in management

oversight,

licensed operator requalification training effectiveness,

and

configuration control.

Another strength of operations

was the questioning

attitude in tracking

and monitoring plant parameters

that allowed early

identification of plant problems,

such

as

steam leaks in containment

and the

clogging of a containment

pressure

sensing line.

Naintenance

was rated category 2.

The plant was generally well maintained

and

long-term equipment

upgrades

continued.

Naintenance

procedures

were very good

following completion of a five year procedure

upgrade

program.

Although most

maintenance activities were performed well, significant exceptions

occurred

during equipment installation or reassembly.

These

problems indicated

weakness

in the efforts of the Naintenance

Department to prevent

and correct

deficient conditions, especially during outages.

Root cause

determinations

associated

with maintenance

problems varied in quality.

Engineering

performance

was rated Category l.

Engineering

Department

management

provided strong oversight in controlling configuration changes

as

evidenced

by the excellent planning

and installation of plant modifications.

The engineering

organization

demonstrated

a strong safety perspective

in

support of plant operations,

including resolution of past technical

problems.

Communications

between the corporate

and site engineering staffs were

excellent.

The Engineering

Department

demonstrated

an excellent root cause

and corrective action program.

Corporate engineering

provided outstanding

support in planning for the steam generator

replacement

program,

conversion to

standard

technical specifications,

and

new fuel reload in 1996.

Plant Support

was rated Category 2.

The plant support functions effectively

contributed to safe plant operations.

Performance

in the radiation protection

area

was good.

Steady

improvements

have

been

made in the

ALARA program

performance,

but areas for growth still exist in meeting exposure

reduction

challenges.

Very good performance

was noted in the radiological effluent and

environmental

monitoring programs.

There

was continued

good performance in

the emergency

preparedness

area,

however several

attention to detail

problems

were identified.

Security program performance

continued to be

a strength,

however

a significant vulnerability was identified in implementation of the

fitness-for-duty program.

The fire protection

and housekeeping

programs

were

effectively implemented.

Enclosure

2 provides the schedule

and basis for NRC inspections of your

facility planned for the next year.

We will inform you of any changes

required to the attached

inspection schedule.

The routine resident

inspection

effort is not included in this schedule.

We have scheduled

a management

meeting with you,

open for public observation,

at ll:00 am on Hay 9,

1995 at the Ginna Training Center.

At the meeting,

you

should

be prepared to discuss

our assessment

and any initiatives you plan to

take to address

the weaknesses

detailed in our evaluation.
