SUBJECT:
NRC INSPECTION 50-244/94-12
This letter transmits the May 3 to June 6, 1994 inspection findings of Messrs. T. Moslak and
E. Knutson at the R. E. Ginna Nuclear Power Plant. At the end of the inspection, the findings
were discussed with Mr. J. Widay and other members of your staff.
The inspection was focused on areas relevant to public health and safety and examined
the
control of operations,
maintenance,
engineering,
and plant support activities coriducted during
this period.
Within this scope,
a number of deficiencies
were identified that related
to
maintenance activities, particularly closure of work packages
and installation of flexible hoses.
While none of these deficiencies resulted in conditions that compromised public health and
safety, they did, in several instances, represent weaknesses
in performing maintenance.
In each
case, we noted that management
has aggressively pursued corrective action; your extensive use
of root cause analyses and human performance system evaluation reviews were noted strengths.
Some of the associated
corrective action evaluations were still in progress
at the close of this
inspection.
The "B" standby auxiliary feedwater pump room cooler was inoperable for 16 days due to
incomplete restoration from maintenance.
Failure to promptly identify this condition represented
a violation of regulatory requirements
as identified herein.
Th'e violation is not being cited
because
the criteria specified in Section VII.Bof the Enforcement Policy were satisfied, in that
the condition had minor safety significance, was self-identified, and was expeditiously corrected.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure willbe placed in the NRC Public Document Room.
