Dear Dr. Mecredy:

SUBJECT:

NRC INSPECTION 50-244/94-09

This letter transmits the May 2-6, 1994 inspection findings of Messrs. E. Knutson and

D. Lew at the R. E. Ginna Nuclear Power Plant.

At the end of the inspection, the findings

were discussed

with Mr. J. Widay and other members of your staff.

The inspection examined areas relevant to public health and safety and was focused on your

corrective action processes.

Overall, these processes

were found to be effective.

Individual

deficiencies were appropriately addressed.

Management involvement in significant events

was excellent, typically resulting in good action to prevent recurrence

and thorough event

analysis.

The use of root cause determinations was excellent.

Additionally, a good initiative

to improve trending of lower-significance events was under development.

Much of the success of your corrective action program was attributed to the individual

performance of your experienced staff and good communications.

However, this high

reliance on the staff reduces the program's defense-in-depth.

The lack of rigorous

documentation throughout the course of corrective action, along with weak guidance as to

what constitutes various types of deficiencies, hinders the identification of weaknesses

in the

program (since much of the supporting information is lost with time) and contributes to

unequal achievement of management

expectations.

Additionally, weak implementation of

management

expectations

and a lack of documentation could result in reduced effectiveness

of the trending program.

Although these problems have not resulted in significant

performance problems, they have detracted from overall program effectiveness.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosure willbe placed in the NRC Public Document Room.
