WESTINGHOUSE REASSESSMENT OF HPA X802 (Bulletin 88-02),

R.E.

GINNA NUCLEAR POWER PLANT (TAC NO. M67308)

R.E.

Ginna was identified by Westinghouse as one of the plants requiring an update of the steam generator tube fatigue evaluations performed by Westinghouse in response to NRC Bulletin 88-02.

The update was required as a

result of NRC staff concerns regarding the accuracy of the anti-vibration bar insertion depth estimates and the associated flow peaking factor estimates.

Your letter of March 2,

1992, provided the reassessment which identified two additional tubes in both steam generators "A" and "B" which required preventative action.

You advised that all of the identified tubes have been plugged with leakage limiting plugs.

During my telephone conversation with Hr. George Wrobel of your staff on December 4,

1992, it was confirmed that the following steam generator tubes were plugged during refueling outage

¹22 (March 27, 1992 Hay 7, 1992),

Steam Generator "A" tubes R12Cll and R13Cll; Steam Generator "B" tubes RllC46 and R13C89.

Additionally, the following steam generator tubes were previously identified as requiring action, and were repaired during refueling outage

¹21 (March 22, 1991 - Hay ll, 1991),

Steam Generator "A" tubes R13C5 and R13C6; Steam Generator "B" tube RllC73.

The staff concludes that your actions resolve the issues identified in Bulletin 88-02 and are, therefore, acceptable.

This finding is subject to the development of administrative controls by you to ensure that updated stress ratio and fatigue usage calculations are performed in the event of any significant changes to the steam generator operating parameters (e.g.,

steam pressure and flow, circulation ratio) relative to those assumed in the WCAP analysis.

Implementation of the additional administrative controls may be subject to verification by an NRC staff inspection.

If, for any reason, you do not plan to implement such administrative controls, please advise us within 30 days of receipt of this letter.
